Instructions:

TO SEND AS AN EMAIL ATTACHMENT:
Print out the [.doc] for the appropriate form.
To mail it back as an email attachment, fill out the form using Microsoft Word or word processing program. Save the file on your computer (remember the file name and saved location!). Go to your email program and attach this file to the email before you send it. Send it to the appropriate person at their email address at the bottom of this page.

TO SEND AS A FAX TO THE CHURCH:
If you cannot send this as an email attachment, print out the appropriate form, fill it out either using a word processing program such as Word [.doc], or by hand (use either [.doc] or [.pdf]), and fax it or bring it to the Church Office at 860.293.1404
